About the Info Sheet
First off, to see the info sheet and licenses more clearly, copy and paste them into word. As I'm a neophyte at web sites, I do not know why my site shows a more blurred view of these sheets and licenses than the real life documents. I'll correct it as soon as I figure it out. The hard copies will look as they do when you paste them into word.
The Info Sheet is a copy of a letter I wrote to a lifelong friend of mine, great classical guitarist and budding songwriter. I gave him as much info as I could about the songwriting aspect of the music business in the least amount of space possible (5 pages). The letter / info sheet includes information on how to copyright songs or groups of songs, starting a publishing company, royalty rates, song administration, sales monitoring and more. I would have also suggested he contact TAXI, but at that time (and even now) I was hearing mixed reviews about them . Also, if you have had anything published at all Nationally you can contact New On the Charts for music industry contacts if you want to do all your own foot work. In any case it 's nice being armed with copyright and publishing info.
About the Licenses
On this site you can only see the 1st page of each available license / agreement. To view them as you will receive them, copy them and paste them into to word. Depending on which License you choose, you will receive either 1,2,3,4,5,6 or 7 pages. These Licenses are intended for you to study but can be printed in Word and used. I was told from just about every attorney, each situation can be a little different. There are different areas to negotiate such as, durations, fees, territories, and more. Any standard, pat or rote contract may not suit your situation. If I were using a license from this or any other web site as the basis of one of my own licenses, I would, after drafting it up, take it to an attorney knowledgeable in the applicable area of entertainment law for which the license was drafted and have them review it for final editing. I personally found it very difficult to find a competent attorney in entertainment law. I would get a referral from someone who has already verified an attorney's competence in the field. This is what I would do and is not necessarily what you should do or could do. I was fortunate enough to be in circles where I had associations with some high profile entertainers. You may not be so lucky and not have such access to word of mouth referrals.
